This is to certify that it appears by the Muster Rolls, filed in this office of the Accountant of the War department that Thomas Fream a Serjeant in Captain Sparln's Company of the Third Sub legion of the United States was discharged on the ninth day of April one thousand seven hundred and ninety five.
C. Brown, John. N. Scott, and R. Allison, Army Surgeons, certify that the said Thomas Fream produced to them a certificate, by which it appears that he was wounded by the Enemy, while in the execution of his duty in public service. They further certify that they have examined said wound, viz. - a ball pafsing passing through the right Lobe of his lungs, by which his respiration is much impeded in bodily exertions - thereby depriving him of the means of acquiring a Subsistence by labour - they therefore think him entitled to a compensation equal to three fifths of that allowed for the highest disability.
Wherefore he is entitled to receive an annual allowance at the rate of three dollars per month, during the continuance of his disability aforesaid; of which he is to produce every six months a certificate of the two respectable Physicians on oath: said pension to commence from the time of his discharge?
Given at the War Office of the United States, this fifteenth day of May, one thousand, seven hundred and ninety seven.
