I certify that Andrew Lewis served as a soldier
in a Company of Volunteer Militia under my
command raised for the protection of the
Inhabitants of the Commonwealth of Virginia
in the frontiers of Kanhawa County in the
year one thousand seven hundred & ninety three
and that by my command he went on express
to give information to the Inhabitants of
the approch of the Indians, & on that service
was unforunately wounded ^ on the 23rd day
of September year above said which occured
an amputation of his right arm, which
renders him incapable of pursing any kind
of manual Labor
Given under my hand at Point Pleasant
the 9th day of June 1797
