This is to certify, it appears from authentic documents on file in this Office, that Nathan Rawlings Captain of a Company of Kentucky Volunteers (under the command of [Brigadier] General Charles Scott) was wounded in an action with the Indians on the twentieth of August 1794.
[Surgeon] [Shannon] and [James H.] [undecipherable: probable reading Phelps] [Surgeons] certify that they have examined the said Nathan Rawlings and are of opinion that by reason of a ball penetrating the [undecipherable] and fracturing it, he is entitled to a compensation equal to one half of that allowed for the highest disability.
Wherefore, he is entitled to receive an annual compensation at the rate of [undecipherable] dollars per month during the continuance of his disability aforesaid, of which he is to produce every six months a certificate of two respectable Physicians on oath: said pension to commence on the twenty seventh day of October 1794, to which time he received pay.
