This is to certify that it appears from an authentic Certificate of D. Allison Surgeon of the first United States Regiment, that Brian M^cCarty soldier of Captain Smith's company of the second Regiment of Levies, was wounded through his life for arm whilst late on duty against the Indians under the command of Major General St. Clair, and also from the certificate of William Stoppen & James Hutchinson Physicians that the said Sarah M^cCarty has lost the use of his left hand and arm. Also that he is entitled during his disability to an allowance of five Dollars a month in pursuance to an act entitled "An act for making and adding a further Regiment to the military Establishment of the United States and for making further Provision for the Protection of the frontiers" passed the third of March one thousand seven hundred and ninety one. And he is to receive every month the evidence of two reputable Physicians on oath of the continuance of his disability by reason of his said wounds. The said allowance to commence from the thirteenth day of January one thousand seven hundred ninety five the day of him his pay ceasing agreeably to the certificate of the accountant of the War department of this day.
