Sir
I am myself the honor to make this account of the Physician that attended Capt. [undecipherable]'s [undecipherable] which was omitted [thro'] mistake when the [other] accounts were forwarded. I am no judge of the reasonableness of the charges but I am informed that this Physician is a man of the strictest honor. I also enclose Mr. [undecipherable]'s commission who on resigning of his appointment had his place filled up by [undecipherable] [undecipherable]. The latter of which Gentlemen requests that you will be pleased to [undecipherable] to me his commission, which according to [undecipherable] [undecipherable]'s [undecipherable] on [undecipherable] and the [register] already forwarded is to be dated from the same time with that of the other Officers —
